As revealed earlier in their Miami trip, Alex has a full-on school-boy crush on Donna. After waiting patiently for her relationship with Moe to fizzle out (and even taking a punch for it in the process) it's quite clear that the feeling is mutual. She didn't realize how much she cared about Alex until he fainted at the beach and had to be rushed to the hospital. "Sometimes it takes sh*t like that for you to realize how you feel about people, and I realized I really cared about you."
